While YAPI comes pre-loaded with the basic forms we think you'll need, you can also download additional pre-made forms from our Forms Library or ask that we make a custom form for you.  If you want to import a pre-made form or have received an email from us that your custom form has been completed, follow the steps below to download and import it.

Downloading a Pre-Made Form from Our Library

1. Visit our Forms Library. Use the Search bar to search for a specific form or use the filter options to find groups of forms like consents, pediatric forms, or new patient paperwork.

   

   Note: You must be a registered Help Center user to access the template links above. Use the Sign In button in the upper right corner of the Help Center and sign in using Facebook, Google, or the Sign Up link.

2. Click the form you want to download.  

3. When a new page loads, showing you what the form looks like, scroll to the bottom.  Click the orange form name to automatically download it to your computer.

   

4. Next, go to the Importing Forms section below to import your form into YAPI.

 

Downloading a Custom Form You Requested

1. If you requested a custom form from our team, you'll receive an email once your request is complete.  Look for an email with the subject line "Your Documents Have Arrived!"

   

2. With the email open, look for the attached .zip file that starts with "FORM_00" and click it to download the file to your computer.

   

   Note: If you requested multiple forms, you may still receive just one file.  It contains all the forms so you can import them all at once!

3. Next, go to the Importing Forms section below to import the file into YAPI.

 

Importing Forms into YAPI

1. From the YAPI Dashboard, click Setup & Manage and select Manage Documents and Templates.

2. When prompted, enter your YAPI password.

3. Click the iPad Forms tab, then click Import. 

4. Find the .zip file you downloaded.  Select it, then click Open.

5. When a prompt pops up asking if you want to continue, click Yes.
