How do I Submit Forms to be Made in YAPI?

If you need to have any consent forms or policies made available on your iPad, these steps outling how to send the form to YAPI's Document Team from your YAPI Workstation.

  1. Select the Help Icon ( Life Preserver or the white and red striped donut ).

  2. Select Request Support.

  3. In the pop-up window, use the pull-down menu to select I am requesting custom iPad or online forms.

  4. A pop-up window will appear allowing you to ADD the requested forms.

  5. Once the requested forms are Added, click the Upload button.

Important:
  1. Please make sure that you are no longer editing and the form is closed before uploading.

  2. Please submit the form in a text selectable format. No Scans or images of forms. In short, If we can highlight the text, it is good to go.

  3. Due to some limitations of the Practice Management software themselves, some forms have limited editing capabilities.
