Adding a Signature to an iPad Form

On occasion, you may need to add a signature to a form a patient has already submitted. For example, when reviewing their Medical History, you may need to add a provider signature or have the patient sign off on any notes you've added. If you or a patient needs to add a signature to a form they've already submitted, follow the instructions below. Â

Adding a Signature to a Completed Form

From the YAPI iPad Dashboard, swipe across on a patient to get to their Forms & Documents section.

Tap the form you want in the Current Documents section.

When the form opens, tap and hold on the form where you want the signature to go.

In the signature box that pops up, add your signature and name or instruct the patient to do so. Then tap Use.

Optional: Add additional signatures using the same steps.

Tap the orange More tab on the left and choose Save.
