Online Forms: Patient Information Update and Medical History

YAPI's New Patient Online forms can be used as a standalone form or be part of an online package. These forms, like all other YAPI forms, use responsive design, which means it is smart phone and tablet friendly, and supports devices with large and small screens, adjusting elements accordingly.

This article covers two types of new patient online forms; The Patient Information, and Medical History forms. These two forms are PMS integrated, which means the information entered in them can be used to update patient records in your Practice Management System.

For any online form, the patient navigates between pages by clicking the Continue and Previous buttons. Before the form is submitted, a review screen pops up allowing the patient to verify their entered information.

Required fields are marked with a red asterisk. Patient must complete all required fields before moving to the next page. 

The page displays your logo on the top and the link to your Practice website at the bottom.

Now let's take a tour of these forms. 

The Patient Information Form

The New Patient Online Form consists of 1 page; a page that collects from the patient their personal and contact information. Notice at the end of the form is a signature area. Patients can use a mouse on PC, or sign with their finger on Mac, a tablet, or a smartphone.

You may notice that the form may show fields when additional information is required. For example, if the patient selects "Other Patient" as a referral source, the form requires patient's name.

After filling out and submitting the form the patient gets a review page. This page allows the patient to verify their input, and submit the form.

When the form is submitted, YAPI presents the following confirmation screen. If the form is a part of a package, YAPI directs the patient to the next form.

The Medical History Form

The Medical History Form consists of 1 page; a page that mainly collects from the patient their medical conditions, allergies and medications they take. The lists are pulled from your Practice Management Software. Note that the patient can add any medical conditions, allergies, and medications that are not listed. Notice at the end of the form is a signature area. Patients can use a mouse on PC, or sign with their finger on Mac, a tablet, or a smartphone.
