Recommended List of Medical Conditions, Allergies, & Medications

When a patient completes their Medical History form, YAPI can integrate their medical information when you file their form into Dentrix, Open Dental, or Eaglesoft. To update the list of Medical Conditions, Allergies, and/or Medications that show on the form, you need to change your lists within Dentrix, Open Dental, or Eaglesoft and then make updates within in YAPI. We've provided our recommended lists to get you started.

Tip: Except for hyphens, always remove punctuation and numbers from the names of your medical conditions, allergies, and medications.  These can cause an error when importing your lists into YAPI.

Table of Contents

Medical Conditions

Note: If you're using Dentrix G5 and above or Open Dental, make sure there is a Medical Condition called "Other" in your list.  When patients are asked if they have a condition that's not on your list, YAPI can then integrate the condition in the patient's chart in Open Dental or their Health History in Dentrix.
