Informed Consent to Perform Dentistry

This form explains the risks, benefits, consequences, and alternatives to general dentistry treatments and oral surgery procedures including examination, x-rays and other diagnostics aids, drugs, and sedation, oral prophylaxes (cleanings), fluoride applications, sealants, restorations (amalgam or composite fillings and crowns), periodontal (gum) treatments, endodontic (root canal) procedures, and extractions. The form also obtains consent from the patient to perform them.
