What are Forms?

YAPI uses a number of different forms types to collect information from your patients. The types of forms we offer are:

  • Online forms are forms you can send your patients to fill in at home. They open up in an Internet browser. They are typically the forms you send to your new patients before they arrive for their first visit. This helps reduce the amount of time you have patients sitting around in your waiting room and can reduce, if not eliminate, errors in entry.

  • iPad forms are forms that can be filled in from the iPad devices available at your practice. Using electronic forms can help reduce pre-procedure anxiety, improve the patient experience, and can also reduce, if not eliminate, errors in entry.

  • PMS-integrated forms are iPad or online forms that are directly integrated with your Practice Management Software (PMS). Examples of such forms are the Patient Information and Medical History forms. These forms ensure that patients' personal information and medical information are appropriately marked within the patient's file in your database.
