Covid-19 - Screening Online Questionnaire

The Covid-19 Screen Questionnaire is intended to assist offices to identify patients that may have been infected due to the spread of the coronavirus. This variant of the Covid-19 Screening Questionnaire is accessible from within YAPI and can be applied to preexisting and new online packages.  Making the form available online can also assist in mitigating any unnecessary exposure to Covid-19 as self-isolation is in effect.

How to Apply Covid-19 - Screening Questionnaire Form

  1. To apply and use the Covid-19 Screen Questionaire, open the YAPI Dashboard.

  2. Click the Setup and Manage icon 2019-09-03_11-44-13.png at the top of the YAPI Dashboard and click the Manage Documents and Templates option from the dropdown.

  3. When prompted for a password, the password by default should be set as "password" all lower case. 

  4. Within the Manage Documents and Templates window, click on the Online tab.

  5. On the bottom-right hand side of the window, click the last option in the list of buttons labeled Get. This will populate the existing list of active online forms.

  6. At the top of the list, the Covid 19 Questionaire will be available. Click on the form from the list, then from the dropdown click on the Install locally and initialize for use option. If a green checkmark is being shown on the left-hand side of the form name, the online form is now available.
