Finding Online Form and Package Links

Individual and packaged online forms all have a unique website URL that is associated with the Practice that generated the forms. If for whatever reason the Practice requires these forms to be signed via email or through an online portal, the URL can be accessed from within YAPI.

The following article will show how to access, copy, and paste the URL for the online forms used by the Practice within YAPI.

Accessing Online Forms


  1. To access the Online Form links, open the YAPI Dashboard.

  2. Click the Setup and Manage icon 2019-09-03_11-44-13.png at the top of the YAPI Dashboard and click the Manage Documents and Templates option from the dropdown.

  3. Enter your password and click Ok when prompted.

  4. Within the Manage Documents and Templates window, click on the Online tab.

  5. On the bottom-right hand side of the window, click the last option in the list of buttons labeled Get. This will populate the existing list of active online forms.

NOTE: Forms that have green checkmarks next on the left-hand side mean they are currently being used in an Online Package. 

Single Online Forms


  1. Click on an Online Form to open a drop-down with a  list of options.

  2. Click on the Show public link option, this will open a pop-up window that will show the URL associated with the Online Form. 

    NOTE: Forms thats are in the green boxes are forms that were pushed online from the unique custom forms found in the iPad Forms tab.


    • To Clipboard - The To Clipboard button allows the Online Form URL to be saved on the clipboard of the workstation. This can allow the user to paste the URL in other mediums. 

    • Launch - The Launch button will open the default web browser of the workstation to view and use the document.

    • Email - The Email button will open a window to send the link of the document via the YAPI email application. 

Online Packages


  1. From the list of buttons on the right-hand side of the window, click on the Packages option, then from the drop-down, click on the Single patient option.

  2. A new window will appear with a Document Package pop-up window in the center. Within the Document Package pop-up window, click on the Open existing package option.

  3. From the drop-down found on the right of the Open existing package option, click and search for the package that needs to share its URL. Then, click the Ok button.

  4. The Online Document Package window will show the contents of the package on the right-hand side, and the URL for the package will be visible and clickable on the bottom of the window. The URL can be highlighted and copied by using the Ctrl + C command. Clicking on the URL will view and use the package.
