What is the Form Builder?
The Form Builder is YAPI's tool for editing and creating custom paperless forms. Within the Form Builder, you can add, edit, and delete questions, adjust formatting, and change your form's file name and title header.

Why Use the Form Builder?
While YAPI comes preinstalled with our stock new patient paperwork and consent forms, we know that every practice is different. With the Form Builder, you can customize these forms to fit your practice's needs and create new forms from scratch.Â
Warning! While basic form editing is intuitive, DO NOT edit Patient Information or Medical History forms without first reading our articles on Creating a Custom Patient Information Form and Creating a Custom Medical History Form. These forms have special instructions because they integrate data into Dentrix, Open Dental, and Eaglesoft.

Accessing the Form Builder
To access the Form Builder, you can open an existing form to edit it or create a new form. Â
When Editing a Form
- From the YAPI Dashboard, click Setup & Manage and select Manage Documents and Templates.
- Click the iPad Forms tab.
- Click the form you want to edit and then click Edit.
When Creating a New Form
- From the YAPI Dashboard, click Setup & Manage and select Manage Documents and Templates.
- Click the iPad Forms tab.
- Click New.

Title
The title that's visible to your patients on iPad and online forms.
Name
The file name for your form that's visible in your list in the iPad Forms tab of Manage Documents and Templates.
Compact
Checking this box reduces the height of input boxes on the form, which will allows more questions to show on each page.
Multiedit
Warning: Do not select this option. It is no longer a working feature.
Scrollable Single Page
If checked, your form becomes a single page that patients can scroll down to complete. Leaving this box unchecked breaks your form up into multiple, shorter pages and patients can click the Next and Back buttons to complete the form.
PMS Integrated
When this box is checked, it indicates the form is data-integrated, which means the form writes information into your practice management software (PMS) - Dentrix, Open Dental, or Eaglesoft. This setting is for Patient Information and Medical History forms only. To read more about data-integrated forms, see our Introduction to Data-Integrated Forms.
More Options
Clicking this button displays language and country options in a pop-up window. These settings are rarely changed.
Question Area
This area displays all the items that show on your form including questions, paragraphs, and signature boxes and formatting elements like section headers and fillers for spacing. For a complete list of items that can be added here, see Questions & Formatting Elements in the Form Builder.
iPad Preview
This preview area displays a rough outline of how your form is formatted, with each element shown as a colored box. Hovering over a box on the iPad Preview highlights the element that box corresponds on in the Question Area.
Note: Because the preview can only show single pages, this view only works for for non-scrollable, single page forms (Scrollable Single Page is unchecked). You can jump between the different form pages by clicking on the next > and back < buttons at the bottom of the previewer:

Delete
Clicking on an item in the Question Area and then clicking this button deletes the item you selected.

Add Multiple
Clicking this button opens a pop-up where you can add multiple items to your form at once instead of selecting each type one at a time. This option is best when creating forms from scratch.

 Add
Clicking this button opens a pop-up where you can select a single item to add to your form. This option is best when editing an existing form that needs a single question or formatting element added.
Â
 Save
Clicking this button saves your form with all its modifications.
