Optional:

  • Choose Has Border to show a border around the question (iPad only).

When using this question type, select which information you want to appear in the question in addition to the patient's address:

  • Add phone (home phone)
  • Add email/mobile phone
  • Add work phone

Optional:

  • Select whether you want the answer to be required.
  • Choose Has Border to show a border around the question (iPad only).
  • Not recommended: Choose Empty if this is a Patient Information form and you don't want fields to autofill with the patient's current information from Dentrix, Open Dental, or Eaglesoft.   If you choose Empty, patients will have to fill out their contact information from scratch every time they update their form on the iPad.

Note: Read Only, With Prefix, and Spanish are no longer working features for this question type.  Selecting either of these boxes has no affect on the appearance of the question or its answer options.

Patient Birthday, Age, Weight

blackX.png  Do not use this question type.  It is no longer a working feature and can negatively affect the integration of your forms.

Patient Header

Screen_Shot_2021-10-18_at_11.27.55_AM.png

Adding it in the
Form Builder:

Screen_Shot_2021-10-18_at_11.00.52_AM.png

When adding this question type, enter gender options under Gender and marital status options under Family Status.

Note: While YAPI can collect as many options as you list for gender, Dentrix, Open Dental, and Eaglesoft have limitations that don't allow integration of options beyond Male and Female when filing the form.

Also, select whether you want the question to also ask for:

  • Patient's middle initial
  • Patient's suffix like "Jr."
  • Patient's preferred name
  • Patient's title like "Mr." or "Dr."
  • Patient's Legal Name
  • Patient's SSN
  • Patient's Driver License number

Note: To ask for a patient's title, simply check the Title checkbox.  Do not enter information in the Title field; this has no affect on the question.

Patient Name

Use This For:

Gathering a patient's name so it appears on the document and can be properly filed into Dentrix, Open Dental, or Eaglesoft.

What it Looks Like:

Screen_Shot_2021-10-18_at_11.28.02_AM.png

Adding it in the
Form Builder:

Screen_Shot_2021-10-18_at_11.01.22_AM.png

Warning! Always add this question to the top of every form except the Patient Information form (it's already included in the Patient Header there).  Also, always select Is Required. Failing to add the Patient Name to a form can make it difficult or impossible to file away.

Optional:

  • Choose Has Border to show a border around the question (iPad only).
  • Select Include Birthday to ask the patient to also enter their birthdate.

Referred By (Open Dental & Eaglesoft Users Only)

Screen_Shot_2021-10-18_at_11.02.21_AM.png

When adding this question type, select the pre-written question "Who may we thank for referring you to our practice?" or type in your own form of the question.

You can also select whether you want the answer to be required.

Note: This question can stand alone as a short answer or can integrate with a list from Open Dental or Eaglesoft.  For more about using an integrated list, see Integrating Your Referrals List into Your Patient Information Form.

Segmented

What it Looks Like:

Screen_Shot_2021-10-18_at_11.28.21_AM.png

Adding it in the
Form Builder:

Screen_Shot_2021-10-18_at_12.02.52_PM.png

When adding this question type, enter the text of your question.  Under Selection Options, list the options you want patients to select their answer from.

Optional:

  • Select Inline for the answer options to appear on the same line as the question instead of beneath it (iPad only).
  • Select whether you want the answer to be required.
  • Choose Has Border to show a border around the question (iPad only).

Note: Marking Small is  no longer a working feature and has no affect on the appearance of the question or its answer options.

Signature

Use This For:

Adding a signature box to the form along with the signer's name and relationship to the patient.

What it Looks Like:

Screen_Shot_2021-10-18_at_11.28.30_AM.png

Adding it in the
Form Builder:

Screen_Shot_2021-10-18_at_11.06.24_AM.png

When adding a signature box to a form, under Footer, enter whose signature goes in that box - for example "Patient Signature" or "Provider Signature."

Optional:

  • Select whether you want the answer to be required.
  • Select Add Name for the signer to type in their name and select their relationship to the patient if the patient isn't completing the form themselves.

Warning! DO NOT use special characters or punctuation in the Footer.  Using these will cause the form to become unusable and uneditable.  For example, enter "Doctor Signature instead of Doctor's Signature."

Single-Line Text

Use This For:

Open-ended questions where a patient may have a short answer of a few words or less.  If this is not a Patient Information form, you can also use this question type to gather a date, a number like an insurance ID, or a patient's phone number, email, or SSN.

What it Looks Like:

Screen_Shot_2021-10-18_at_11.28.38_AM.png

Adding it in the
Form Builder:

Screen_Shot_2021-11-05_at_9.03.38_AM_2.png

When adding this question type, enter your question and in the Input Type pulldown menu, select what form you need the patient's answer to be in:

  • Text (default)
  • Numeric only (forces a number keyboard to appear instead of a letter keyboard and patient can't move on in the form until only numbers are entered, iPad only).
  • Numbers (forces a number keyboard to appear instead of a letter keyboard, iPad only).
  • Date (on the iPad, patients can select a date from a calendar; online, patients are presented with boxes for Month, Date, and Year)
  • Email
  • Phone
  • SSN

Note: Never use this question type with the label "First Name" or "Last Name" on Patient Information or Medical History forms. Do not use the autocapitalized text or mmyyyy options.  Autocapitalized text is no longer a working feature and mmyyyy can cause an error on the form.

Optional:

  • Select Inline for the answer options to appear on the same line as the question instead of beneath it (iPad only).
  • Select whether you want the answer to be required.
  • Choose Has Border to show a border around the question (iPad only).
  • Enter Placeholder text as an example answer that appears in grey within the answer box and disappears once a patient starts typing in it (iPad only).

Note: Initial Value and Read Only are no longer working features.  Adding text to the Initial Value field or selecting Ready Only has no effect on the question or answer options.

Use This For:

Gathering a patient's student status.

What it Looks Like:

Screen_Shot_2021-10-18_at_11.28.46_AM.png

Adding it in the
Form Builder:

Screen_Shot_2021-10-18_at_11.10.38_AM.png

When adding this question type, you can:

  • Edit the Options for student status.
  • Select whether you want the answer to be required.
  • Select Include School Name question to also have the patient provide their school's name.

Note: On Patient Information forms, it's best that this question appear as far down on the form as possible to avoid issues with the form integrating into Dentrix, Open Dental, or Eaglesoft.

Web Popup (for iPad Forms Only)

Use This For:

Adding a popup link within an iPad form so a patient can view a webpage before completing their form.

What it Looks Like:

Screen_Shot_2021-10-18_at_11.32.35_AM.png

Adding it in the
Form Builder:

Screen_Shot_2021-10-18_at_11.30.34_AM.png

When adding a Web Popup, enter the text you want patients to click on in the Title section.  Select Internet and enter the web address of the webpage you want to pop up when the patient clicks on the Title text.

Webpage

blackX.png  Do not use this question type.  It is no longer a working feature and no longer appears on forms when added.  If you need to add a webpage address to a form, use Web Popup for iPad forms or enter the web address in a Paragraph for online forms.

Use This For:

Questions that need a yes or no answer.

What it Looks Like:

Screen_Shot_2021-10-18_at_11.28.57_AM.png

Optional:

  • Select whether you want the answer to be required.
  • Choose Has Border to show a border around the question (iPad only).
  • Include the option of requesting a followup answer under Detailed Answer.  You can make this followup required and have it appear either when a patient selects Yes or when a patient selects No.

When adding Filler, we recommend setting the Height at 50 and making adjustments after previewing your form on the iPad.

Use This For:

Adding a heading to the form or a section of the form.

What it Looks Like:

Screen_Shot_2021-10-19_at_10.06.54_AM.png

When adding a Header, enter the text of your header and select the Text Size, Alignment, and Text Color.

Optional:

  • Uncheck Transparent and double-click Background Color to set a background color for your text.
  • Select whether you want this Header to appear on the iPad version of this form, the online version, or both.

Image (for iPad Forms Only)

Use This For:

Adding an image to an iPad form.

What it Looks Like:

Screen_Shot_2021-10-19_at_10.07.50_AM.png

Adding it in the
Form Builder:

Screen_Shot_2021-10-19_at_10.04.52_AM.png

When adding an image to your form, you need to host your image online first.  Then, enter the image's URL and set its Width and Height.

You can also:

  • Change the X Offset to shift the image farther to the right.  To center an image, we recommend starting with 100-200 as an X offset and adjusting it  after testing the form on your iPad.

Note: Images will not appear on the final filed PDF copy of the form.  Also, Action Script is no longer a working feature.  You can leave this space blank. Also, these images can only appear on iPad forms, so selecting Online or Both has no affect on whether the image can be viewed on online forms.

Label

blackX.png  Do not use this formatting element; it's no longer a working feature.  To add a title to section, use a Header instead.

Page Break (for iPad Forms Only)

What it Looks Like:

Screen_Shot_2021-10-19_at_10.19.34_AM.png

Adding it in the
Form Builder:

Simply click Add and select Page Break from the dropdown menu.  There is no popup window for this item.

If this form will be used both on your iPad and online, we recommend also adding a Page Break for Online Document element.

Warning!  Do not add page breaks to a form that has Single Scrollable Page selected.  This can prevent patients from seeing all pages and completing the form.

Screen_Shot_2021-10-20_at_8.14.04_AM.png

Page Break for Online Document (for Online Forms Only)

Use This For:

Adding a page break to online forms so that the content below the Page Break shows on a new page.  This also adds a progress bar at the top of each page showing how much of the form has been completed.

What it Looks Like:

Screen_Shot_2021-10-19_at_10.24.16_AM.png

Screen_Shot_2021-10-19_at_10.24.37_AM.png

Form Builder:

Screen_Shot_2021-10-19_at_9.54.14_AM.png

When adding a Page Break for online forms, add the page title that will appear above the progress bar on the next page.

If this form will be used both on your iPad and online, we recommend adding a Page Break (for iPad Forms) just above this element.

Warning!  Do not add page breaks to a form that has Single Scrollable Page selected.  This can prevent patients from seeing all pages and completing the form.

Screen_Shot_2021-10-20_at_8.14.04_AM.png

Separator (for iPad Forms Only)

Use This For:

Adding a separating line on a form.

When adding a Separator, set the line's thickness in the Height box.  For example, a height of 1 shows a thin line and a height of 5 shows a thick line.

Optional:

  • Double-click the Color box to change the line's color.
