When creating or editing a form in YAPI, you can add different types of questions and formatting elements to make your form easy for your patients to complete. Below, you'll find a complete list of all the elements you can add to a form in the Form Builder.

Acknowledgement Checkbox
Use This For: |
Statements that you need a patient to acknowledge. |
What it Looks Like: |

When adding this question type, enter the text that you want the patient to acknowledge in the Statement box and check whether you want their answer to be a required item on the form. |
Allergy (Open Dental Users Only)
Use This For: |
Medical History forms for Open Dental users only. This question asks patients to note their allergies and integrates with the Allergies list in Open Dental. |

Appointment Reminders
Use This For: |
Asking patients how they want to receive appointment reminders. Note: The patient's answer doesn't affect what kind of reminders they receive through YAPI. It simply shows their preference. |

When adding this question type, questions are pre-written for you but you can change the wording. Select whether you want the questions to appear on the iPad version of this form, the iPad version, or both. You can also select Spanish to change the answer options on this question from Yes/No to Si/No. |

Questions where a patient should select multiple answer options. |

When adding this question type, enter the text of your question. Under Selection Options, list the checkbox options you want patients to select from. Optional:
Warning! Do not select Use YN Button. This can cause the question to appear incorrectly. Also, selecting Small Font is no longer a working feature and has no affect on the question. |
Data Row (for iPad Forms Only)
Do not use this question type; it's no longer a working feature.

Use This For: |
Questions where a patient should select one answer option from a pre-determined list. |

Medical Conditions (Dentrix & Open Dental Users Only)
Use This For: |
Medical History forms for Dentrix and Open Dental users only. This question asks patients to note their medical conditions and integrates with your list of medical conditions in Dentrix or Open Dental. |

When adding this question type, select a pre-written question from the dropdown list or type in your own form of the question. Select Add "I checked all that apply..." to add a checkbox for a patient to acknowledge they've checked all the conditions that apply to them. |
Medications (Dentrix & Open Dental Users Only)
Use This For: |
Medical History forms for Dentrix and Open Dental users only. This question asks patients to note their medications and integrates with your list of medications in Dentrix (G7 and above) or Open Dental. |

Multi-Line Text

Providing text or instructions just below the title of an online form. |

Paragraph
Use This For: |
Paragraphs of text that don't require a response. For example, an explanation of office policies or a description of a procedure. |

Patient Address
Use This For: |
Gathering the patient's address, home phone number, mobile phone number, work phone number, email, and preferred language. |

When using this question type, select which information you want to appear in the question in addition to the patient's address:

Note: Read Only, With Prefix, and Spanish are no longer working features for this question type. Selecting either of these boxes has no affect on the appearance of the question or its answer options. |
Patient Birthday, Age, Weight
Do not use this question type. It is no longer a working feature and can negatively affect the integration of your forms.
Patient Header
Use This For: |
Gathering a patient's full name, preferred name, SSN, Driver License number, gender, and family status on a Patient Information form only. |

When adding this question type, enter gender options under Gender and marital status options under Family Status. Note: While YAPI can collect as many options as you list for gender, Dentrix, Open Dental, and Eaglesoft have limitations that don't allow integration of options beyond Male and Female when filing the form. Also, select whether you want the question to also ask for:
Note: To ask for a patient's title, simply check the Title checkbox. Do not enter information in the Title field; this has no affect on the question. |
Patient Name
Use This For: |
Gathering a patient's name so it appears on the document and can be properly filed into Dentrix, Open Dental, or Eaglesoft. |

Warning! Always add this question to the top of every form except the Patient Information form (it's already included in the Patient Header there). Also, always select Is Required. Failing to add the Patient Name to a form can make it difficult or impossible to file away. Optional:

Referred By (Open Dental & Eaglesoft Users Only)
Use This For: |
Asking a patient who referred them to your practice. |
What it Looks Like: |

When adding this question type, select the pre-written question "Who may we thank for referring you to our practice?" or type in your own form of the question. You can also select whether you want the answer to be required. Note: This question can stand alone as a short answer or can integrate with a list from Open Dental or Eaglesoft. Segmented
Use This For: |
Questions where a patient should select one answer option from a pre-determined list. |

Signature
Use This For: |
Adding a signature box to the form along with the signer's name and relationship to the patient. |

When adding a signature box to a form, under Footer, enter whose signature goes in that box - for example "Patient Signature" or "Provider Signature." Optional:
Warning! DO NOT use special characters or punctuation in the Footer. Using these will cause the form to become unusable and uneditable. For example, enter "Doctor Signature instead of Doctor's Signature." |
Single-Line Text
Use This For: |
Open-ended questions where a patient may have a short answer of a few words or less. If this is not a Patient Information form, you can also use this question type to gather a date, a number like an insurance ID, or a patient's phone number, email, or SSN. |

Note: On Patient Information forms, it's best that this question appear as far down on the form as possible to avoid issues with the form integrating into Dentrix, Open Dental, or Eaglesoft. |
Web Popup (for iPad Forms Only)
Use This For: |
Adding a popup link within an iPad form so a patient can view a webpage before completing their form. |

When adding a Web Popup, enter the text you want patients to click on in the Title section. Select Internet and enter the web address of the webpage you want to pop up when the patient clicks on the Title text. |
Webpage
Do not use this question type. It is no longer a working feature and no longer appears on forms when added. If you need to add a webpage address to a form, use Web Popup for iPad forms or enter the web address in a Paragraph for online forms.
Yes/No
Use This For: |
Questions that need a yes or no answer. |
What it Looks Like: |

When adding this question type, you can enter your question and change the wording of the Yes or No options. For example, if your form is in Spanish you could enter "Sí" or "No" instead. Optional:
Note: Selecting Inline and Small have no affect on this question. Questions and answer options appear the same regardless of these settings. |
Formatting Elements

Filler (for iPad Forms Only)

Adding spacing between questions. |

When adding Filler, we recommend setting the Height at 50 and making adjustments after previewing your form on the iPad. Note: 1000 is the max height you can use for one filler. If you need more space, add a second filler to your form. |
Header
Use This For: |
Adding a heading to the form or a section of the form. |

When adding a Header, enter the text of your header and select the Text Size, Alignment, and Text Color. Optional:

Image (for iPad Forms Only)

Adding an image to an iPad form. |

When adding an image to your form, you need to host your image online first. Then, enter the image's URL and set its Width and Height. You can also:
Note: Images will not appear on the final filed PDF copy of the form. Also, Action Script is no longer a working feature. You can leave this space blank. Also, these images can only appear on iPad forms, so selecting Online or Both has no affect on whether the image can be viewed on online forms. |
Label
Do not use this formatting element; it's no longer a working feature. To add a title to section, use a Header instead.
Page Break (for iPad Forms Only)
Use This For: |
Adding a page break to non-scrollable iPad forms only so that the content below the Page Break shows on a new page. |

Simply click Add and select Page Break from the dropdown menu. There is no popup window for this item. If this form will be used both on your iPad and online, we recommend also adding a Page Break for Online Document element. Warning! Do not add page breaks to a form that has Single Scrollable Page selected. This can prevent patients from seeing all pages and completing the form. |
Page Break for Online Document (for Online Forms Only)
Use This For: |
Adding a page break to online forms so that the content below the Page Break shows on a new page. This also adds a progress bar at the top of each page showing how much of the form has been completed. |

When adding a Page Break for online forms, add the page title that will appear above the progress bar on the next page. If this form will be used both on your iPad and online, we recommend adding a Page Break (for iPad Forms) just above this element. Warning! Do not add page breaks to a form that has Single Scrollable Page selected. This can prevent patients from seeing all pages and completing the form. |
Separator (for iPad Forms Only)
Use This For: |
Adding a separating line on a form. |

When adding a Separator, set the line's thickness in the Height box. For example, a height of 1 shows a thin line and a height of 5 shows a thick line. Optional:
