Renaming & Creating Copies of Forms

On occasion, you may want to change the name of a YAPI form or make a copy of a form to edit.  In YAPI, this is the same process: renaming a form automatically creates a new copy with the new name (similar to Save As in Microsoft Word).  Follow the instructions below to rename your form, creating a new copy that you can edit.

Note: These instructions are for updating the name on iPad forms.  If you've changed the name of your iPad form and you need this form made online to reflect the new name there, contact our Customer Support team.  They can connect and make the form online for you in a couple of minutes.

Names for preinstalled online forms that show in white, pink, or blue in the Online tab of Manage Documents and Templates cannot be changed.  To change the name, these forms need to be reconstructed as an iPad form, then made online with the new name.

Renaming a Form

  1. From the YAPI Dashboard, click Setup & Manage and select Manage Documents and Templates.

  2. When prompted, enter your YAPI password.

  3. In the Manage Documents and Templates window that pops up, select the iPad Forms tab.

  4. Click the name of the form you want to rename or copy.

  5. Click Edit.

  6. In the Form Builder that pops up, change the name of the template in the Name field and click Save.

  7. Optional: Make additional edits to the form, then click Save again.  These edits only apply to the new copy of your form with the new name.  For more information about editing your form, see Editing Existing Forms in the Form Builder.

  8. Close the Form Builder window and click away to another tab within the Manage Documents and Templates window.

  9. Click back to the iPad Forms tab.  Make sure your form with the new name appears here. 

  10. Click the name of the form and click Category.  Select the category that this form will file to in Dentrix, Open Dental, or Eaglesoft when completed by a patient.

  11. Double-click the name and when a popup asks if you want to make the form available for signing, click Yes.

Deactivating the Original Form

  1. If you want to keep the original copy of the form but deactivate it so it doesn't show up on the iPad, navigate to the iPad Forms tab of Manage Documents and Templates.

  2. Locate the original form name in the list.

  3. Double-click the name and when a popup asks if you want to make the form not available for signing, click Yes.

Deleting the Original Template

  1. If you want to delete the older version of your form, navigate to the iPad Forms tab of Manage Documents and Templates.

  2. Click the old form with the old name, then click Remove

  3. When asked if you're sure you want to remove the form, click Yes.
