Categorizing Online Forms

When filing forms, each form needs a Category assigned so YAPI knows where to file it in your practice management software (Dentrix, Open Dental, or Eaglesoft).  To save time when filing, you can categorize all your online forms in advance. Follow the instructions below to categorize your each of your online forms.

Categorizing Online Forms


  1. From the YAPI Dashboard, click Setup & Manage Setup_and_Manage.png and select Manage Documents and Templates.

  2. When prompted, enter your YAPI password.

  3. In the Manage Documents and Templates window that pops up, click the Online tab.

  4. Click Get to load your online forms.

  5. Click the name of the form you want to categorize and choose Select Category.

  6. Select the appropriate Category for the form from the dropdown list and click Ok.

    Note: The Categories that show in this list correspond to folders in your practice management software.  If you're not sure which Category your form should go in, consult your office team or select the category that seems the most appropriate for the form. To change your list of categories, see Setting Up Document Types for Dentrix, Image Categories for Open Dental, Document Groups for Eaglesoft.
  7. Optional: Repeat steps 5 and 6 for all your other online forms.
