Updating the Lists of Medical Conditions, Allergies, & Medications That Show on Your Medical History Form: Open Dental
The lists of Medical Conditions, Allergies, & Medications that appear on your YAPI Medical History forms are generated by you in Open Dental and imported into YAPI. This way, when your patients mark that they have a condition or take a medication, the data inputs easily back into their Open Dental Chart module when you file their form.
These lists are imported by our Technical Support team at installation, but if you ever need to update them, follow the instructions below.

Step 1: Updating Your Lists in Open Dental
Changes to the Medical Conditions, Allergies, and/or Medications that show on your Medical History forms always start in Open Dental. If you've already made these changes, skip to Checking Your Lists in YAPI below.
Changing your list of Medical Conditions in Open Dental:

In the Open Dental toolbar, click Lists.

Select the list you want to edit: Allergies, Medications, or Problems (Medical Conditions).

Add an Item: Click Add, enter it's name into the Description box, and click OK.

Change an Item: Double-click it in the list. In the window that pops up, change its name in the Description box. Then click OK.

Delete an Item: Double-click it in the list. In the window that pops up, click Delete.
For items that Open Dental won't allow you to delete:

Hide them (Medical Conditions or Allergies): Double-click the item in the list and select the Hidden (for Medical Conditions) or Is Hidden (for Allergies) checkbox. Then click OK.

- Click Close to save your changes.
Note: When you change your lists in Open Dental, you may also see this notification in YAPI, recognizing the change:
Step 2: Checking Your Lists in YAPI

From the YAPI Dashboard, click Tools, then select Administrative Tools, then Lists.

From the dropdown:
- Choose Medical Conditions if you updated your Problems list in Open Dental.
- Choose Allergies if you updated your Allergies list in Open Dental.

Choose Medications if you updated your Medications list in Open Dental.

Scroll down to "Other" and make sure the box next to it is unchecked.

Click Save.

If a warning pops up saying you have duplicates, then delete, hide, or merge the duplicates in Open Dental by using the instructions in Step 1 above. Then, come back to check your lists in YAPI again.

If a popup asks if you want to update your online list, click Yes, then Yes again.
You'll know the list is finished updating when the window closes.

Repeat Steps 1-4 if you also updated another list in Open Dental.
Why uncheck "Other"? This automatically tells YAPI to add followup questions to your Medical History forms, where patients can note that they have a medical condition, allergy, or medication that's not on your list. When you file the form, YAPI writes these to the patient's Chart in Open Dental as an "Other" with a note of what the condition is:
Step 3: Testing Your Medical History Form
Before giving your form to patients, we recommend testing it to make sure your updated lists show and that information on the form integrates properly back into Dentrix. To test your form, you need a test patient checked into the YAPI Dashboard.
Checking in a test patient without making an appointment:

From the YAPI Dashboard, click Patients and select Patient Finder.

Enter the name of a test patient from your practice management software and click Search. Most practices have a test patient named Test Test, YAPI Test, or Test YAPI.

Click the name of the test patient and select Check-in.
Testing your form:

From the YAPI iPad App Dashboard, swipe across your test patient. When the Forms & Documents section opens, scroll down to the Forms section and tap on any Medical History form to open it.

When the form opens, review it to make sure the updates to your lists are showing. If they're not, repeat Step 2 above and make sure your lists are fully saved before opening the form again.

Complete the form as the test patient, selecting at least one new medical condition, one allergy, and one medication.

File the form into Open Dental. (If your Medical Histories file automatically using Fast Import, you can skip this step.)

In Open Dental, open the test patient's Chart and make sure the conditions and medications you selected on the form show up. If they show up, you're done!
If they don't show in Open Dental, try restarting Open Dental and testing the form again. If the conditions and medications you selected still don't show, reach out to our Customer Support team. This may indicate an issue with your form or your connection between YAPI and Open Dental.
