Creating Custom Spanish Forms
Using Forms in Yapi Leap (our new web app)? 🤔 In that case, looks like you're in the wrong place. Instead, go to the corresponding topic in our
Leap Help Center about making your forms multi-language:
If you've set up Spanish forms within YAPI, you may find that you want to create a custom form in addition to the pre-made Spanish forms we provide. If you're not familiar with creating custom forms in English, we recommend reading Creating a Basic Custom Form first, then returning here to learn more about creating Spanish forms below.

Before Creating Custom Spanish Forms
Before building your own Spanish Forms, you need to set up YAPI to work with Spanish forms first. This includes importing a translation file, translating your medical conditions and allergies lists (for Dentrix and Open Dental users), and importing our library of pre-made Spanish forms. You can find full instructions for this in our article Setting Up Spanish Forms. Â
This ensures that the labels for certain questions (like name, date, address, etc.) translate into Spanish on your forms and that you have a library of basic Spanish forms to work with.
Â
Starting Your Custom Spanish Form
Creating a Spanish form in the Form Builder is almost the same as creating one in English, with just a couple of differences, outlined in the sections below.Â
-
To start your custom Spanish form, use these articles to guide you:
-
Then, use the instructions below to set the form's language and remove or replace special characters within your form.
If you're creating a custom Patient Information or Medical History form, make sure to use the pre-made Spanish Patient Information and Medical History forms we've provided in our Setting Up Spanish Forms article as a base to edit from. Don't start these forms from scratch.
Â
Setting the Form's Language
-
If the form you started in the previous section isn't open, open it form in the Form Builder by clicking Setup & Manage
in the YAPI Dashboard, selecting Manage Documents and Templates, and entering your YAPI password.
-
Click the iPad Forms tab, then select the form you created from the list. Then click Edit.
-
When your form opens in the Form Builder, click More Options.
-
In the Language menu, select Spanish. Then click OK and Save.
Â
Removing & Replacing Special Characters
-
Replace special characters that appear anywhere other than a Paragraph. YAPI is unable to process Spanish characters like á, é, Ã, ó, ú, and ñ in other types of questions, Headers, and in the Name or Title of the form. Including them can cause the form to read incorrectly or fail to open. Instead, replace these characters with their closest English equivalents a, e, i, o, u, and n:
-
If any Paragraphs contain Spanish characters and Has HTML is selected, you must replace all Spanish characters with the appropriate HTML code. For more on this, including the codes you'll need, see Using HTML to Customize Forms. (If Has HTML is unchecked, you can skip this step.)
Â
Finishing Up
As with any form you work with in the Form Builder, when you're done, click Save, then close the Form Builder. Even if you've saved, a popup will always ask if you want to save before closing the window; click Yes.
Before using your form, you need to enable and test it on an iPad to make sure it's patient-ready. For that next step, see Testing Your Custom Form on an iPad.
Tip: If you want an online copy of the form to send to your patients or post on your website, connect with our Customer Support team once your form is finished and has been tested on an iPad.
