Assigning iPad Forms to Patients

With YAPI's paperless forms, you can easily gather information from patients on an iPad. Follow the instructions below to assign single or multiple forms to a patient. If you need your patient to sign a treatment plan, see our article on Signing Treatment Plans on an iPad.
Note: Your patient must be checked into YAPI to assign them forms on an iPad.

Opening a Single Form to Sign
If your patient only needs to sign one form like a consent or a financial policy:

Open the YAPI iPad app. If you already have the app open, make sure you're on the Dashboard screen by tapping Dashboard in the bottom toolbar.

Locate your patient and swipe across their colored bar (left to right OR right to left).

When the page flips to the Forms & Documents section, scroll down to the Forms section and find the form you want. Then, tap it to open:
- Hand the iPad over to your patient and ask them to return it when they're finished.Â
Note: If your provider needs to sign the form too, ask the patient to simply sign but not click the Save button.

Assigning a Pre-Made Paperwork Package
Our Technical Support team creates several paperwork packages for you at installation so that you can quickly assign new and returning patients the basic forms they need. To assign one of these pre-made packages to a patient:

Locate your patient on the Dashboard of the YAPI iPad app.

Swipe across their colored bar (left to right OR right to left).

When the page flips to the Forms & Documents section, scroll down to the Packages section and locate the package they need. Then, tap it to assign the package.

Hand the iPad over to your patient and ask them to tap on each form to complete it and return the iPad when they're done. They'll know their forms are complete when each one shows a green check mark next to it:

Creating & Assigning a Custom Paperwork Package for a Patient
If your patient needs to sign multiple forms that aren't already in a pre-made package, you can put together an "On-the-Go" package for them or combine a pre-made package with additional forms (like an update package and a single consent form).
