Troubleshooting: Forms Can't Be Saved or Opened on an iPad or Don't Auto-File via Fast Import
In 2018, YAPI introduced new, more customizable paperless forms. Forms constructed in the old form editor before 2018 are now considered "legacy forms" and using them can cause the following errors:
- The form won't open on an iPad
- The completed form won't save on an iPad
- The form won't auto-file with Fast Import
To fix these errors, you need to replace the forms within YAPI. Below, you'll learn how to recognize older forms and how to replace them with new up-to-date ones.

Recognizing Older "Legacy" Forms
If you're unsure whether you have "legacy" forms that need to be replaced:
-
From the YAPI Dashboard, click Setup & Manage
and select Manage Documents and Templates.
-
When prompted, enter your YAPI password.
-
In the Manage Documents and Templates window that pops up, click the iPad Forms tab. Legacy forms appear with uncapitalized names in the list (our newer forms show in all caps):
If you select one and click Edit, they'll appear like the example below:
Replacing Legacy Forms
If you find your practice still has these older "legacy" forms, you have several options for replacing them:
- Download and import our free stock forms
- Convert custom forms on your own
- Gather custom forms for the YAPI team to rebuild
Downloading & Importing New Stock Forms
If you're using an old form that came installed with YAPI and was never customized, just replace it with the new one in our online Forms Library!
-
Navigate to the YAPI Forms Library and use the search to find the new version of the form.
-
Click on the form and scroll down to the bottom of the page. Click the file name to download it:
-
Optional: Repeat with any other forms you need to replace.
-
Once you've downloaded all the forms you need, follow the instructions in Importing Pre-Made & Custom Forms to import the forms into YAPI.
Rebuilding Custom Forms on Your Own
If you have custom legacy forms, they need to be rebuilt since they're specific to your office. You can copy the text of the old form and create a new one using the Form Builder:
-
From the YAPI Dashboard, click Setup & Manage
and select Manage Documents and Templates.
-
When prompted, enter your YAPI password.
-
In the Manage Documents and Templates window that pops up, click the iPad Forms tab.
-
In the list of forms, click the name of the form you need to recreate and click Edit.
-
Highlight the text and click the Copy
button. Then, paste the text into an external word processing program like Google Docs or Microsoft Word.
-
Use the instructions in our article Creating a Basic Custom Form to rebuild your form within the Form Builder. (For Patient Information and Medical History forms specifically, see Creating a Custom Patient Information Form or Creating a Custom Medical History Form instead.)
Gathering Custom Legacy Forms for the YAPI Team to Rebuild
If you're unable to recreate the forms on your own, you can export and submit your old forms to the YAPI team to rebuild for you:
-
From the YAPI Dashboard, click Setup & Manage
and select Manage Documents and Templates.
-
When prompted, enter your YAPI password.
-
In the Manage Documents and Templates window that pops up, click the iPad Forms tab.
-
In the list of forms, click the name of one of your old forms and click Export.
-
In the window that pops up, select where you want to save the form on your computer, then click OK.
-
Repeat with each legacy form you need rebuilt.
-
Send an email to our Documents Team at documents@yapicentral.com noting that the attached files are "legacy forms" you need them to rebuild. Then, attach all the form files you exported in the steps above and send your request! Once finished, our team will email the completed forms to you with instructions for importing them into YAPI.
For more information about turnaround times and what to expect once your forms are rebuilt, see Submitting a Custom Form Request to the YAPI Team.
