Creating iPad Form Packages

Within YAPI, you can create pre-set packages of forms for your patients to fill out on an iPad. Building these iPad packages makes assigning frequently-used sets of forms faster and easier. Below, you'll learn how to create packages of iPad forms that your patients can complete in your office.
Note: During installation, your installer always creates new patient form packages as well as patient update packages to get you started. New patient packages include our Patient Information, Medical History, Dental History, Insurance, and HIPAA Privacy Acknowledgement forms. Update packages each include a Patient Information and a Medical History form.

Creating an New iPad Form Package

From the YAPI Dashboard, click Setup & Manage and select Manage Documents and Templates.

When prompted, enter your YAPI password.

In the Global Setup window that pops up, navigate to the iPad Packages tab.

Click New.

In the Package Name box, type the name of your package.

Add forms to your package by selecting each one you want to add in the blue All Forms list, then clicking the > arrow button to move it to the yellow Package Content list:

Optional: To change the order that forms appear for your patients, click a form you want to move within the yellow Package Content section and drag it up or down within the list.

Click OK, then close theManage Documents and Templates window.
