Finding the Web Link for Online Forms & Packages
Individual online forms and online form packages all have a unique website link (URL) that patients can use to access and submit your practice's online forms. Below, you'll learn how to find the link for a single form or for a pre-made package of online forms that you can then post on your practice website or send to patients via email or text.

Finding the Link for a Single Form

From the YAPI Dashboard, click Setup & Manage and select Manage Documents and Templates.

When prompted, enter your YAPI password.

In the Manage Documents and Templates window that opens, navigate to the Online tab.

Click the Get button to load your online forms.

Scroll through the list to find the form you need a link for.
Important! If the form name doesn't have a green check mark next to it, you must enable the form first. If you generate the link for a form that doesn't have a check mark, the form will not come to your Document Queue when a patient submits it online.

Click on the name of the form and select Show Public Link.

In the Online Package Link window that appears, select:
- To Clipboard to copy the link for this package. Paste it into an email template or text template to send to your patients or send it to your web developer so they can add it to your practice website.
- Launch to open the online form and preview it.
- Email to send an email with the link to this package.

When finished, close the Online Package Link window, then close the Manage Documents & Templates window.
Finding the Link for a Form Package

From the YAPI Dashboard, click Setup & Manage and select Manage Documents and Templates.

In the Manage Documents and Templates window that opens, navigate to the Online tab.

Click the Get button to load your online forms.

Once your list of online forms loads, click Packages and select Single Patient.

In the Document Package window that pops up, select Open Existing Package, then from the dropdown select the form package you want to edit.

Click on the name of the form and select Show Public Link.

In the Online Package Link window that appears, select:
- To Clipboard to copy the link for this package. Paste it into an email template or text template to send to your patients or send it to your web developer so they can add it to your practice website.
- Launch to open the online form and preview it.
- Email to send an email with the link to this package.

When finished, close the Online Package Link window, then close the Manage Documents & Templates window.
