Using YAPI's Free Forms & Templates Library
Your YAPI subscription comes with free access to additional paperless forms, email templates, and other resources that you can download and use with YAPI's features. Use the links below to create a Help Center account, view YAPI's content libraries, and download what you need!

Accessing YAPI's Free Resources
To access all the resources that come as part of your subscription, you need to create a free account on our Support Center. If you don't already have a Support Center account, click to follow the instructions below:
- Navigate to YAPI's Support Center homepage and click the Sign In button:
- In the pop-up, click the Sign Up link in the bottom left:
- Enter your name and email, then click Sign Up.
- Check your email for a message from YAPI and follow the prompts to set a password. Now you have full access to all the resources on the Support Center!
Pre-Made Forms
YAPI's Forms Library includes over a hundred pre-made forms that you can preview, download, and import into YAPI for use with your patients. Here, you'll find specialized consent forms, questionnaires, and post-op instructions as well as all the forms that come installed standard with YAPI.

When you find what you're looking for, just follow the instructions in our article Importing Pre-Made & Custom Forms to download a form and add it to your YAPI.
Need a custom form? You can create your own in YAPI's Form Builder or submit a custom form request to our team!
