To send online forms to your patients, all you need to do is find the link to the form or form package they need and either email it or text it to them:
-
To find the link for a form or form package, follow the steps in Finding the Web Link for Online Forms & Packages.
Note: When following these directions, if you don't find the form or package you need to send, this means it's not available for online use. To make it available online, contact our Customer Support team. Making your form(s) available will only take a few minutes and then you can continue on to the next step!

To create a template to send to patients later:
Tip: Creating a template or canned message is a great way to save your team time when you send out the same forms to patients regularly. For example, if you find that you often have to send existing patients an update package or send individual patients a form to update their insurance information, we definitely recommend making a canned message or template so you don't have to send the message or find the link again!
